Safety Net Medical Home Initiative

Supported by The Commonwealth Fund and in conjunction with Qualis Health, ICIC is working to bring the patient-centered medical home to 50 clinics caring for underserved or economically disadvantaged U.S. communities Qualis Health and ICIC staff are providing technical assistance to clinical practices in four geographic regions over a five-year period. 
 
Forty-two organizations from 31 states applied to become part of the Safety Net Medical Home Initiative. Five Regional Coordinating Centers (RCC) were selected to participate in the demonstration project, and each partnered with 12-15 safety net clinics in their state.  The RCCs are: 
These collaboratives will receive technical assistance on practice re-design topics such as enhanced access, care coordination and patient experience. They will also receive funding to support a Medical Home Facilitator (who will lead clinic-based quality improvement projects) and other activities. The work of the Regional Coordinating Centers began in April 2009 and the Initiative will continue through April 2013.
