Improving Chronic Illness Care

Patient-Centered Medical Home Assessment

The MacColl Institute, in partnership with Qualis Health, is leading the Safety Net Medical Home Initiative (SNMHI) with the goal of transforming safety net clinics into Patient-Centered Medical Homes.  Early in the study, we identified the need for an assessment tool that could be completed by providers and other members of the care team to measure their progress in the eight change concept areas established as key components of a Patient-Centered Medical Home.

We have modeled the instrument on the Assessment of Chronic Illness Care (ACIC), utilized by hundreds of teams to guide their Chronic Care Model-based improvement efforts.  Similar to the ACIC, each question in this instrument will have four response levels, describing where a team might find itself on the journey to becoming a Patient-Centered Medical Home.

The Safety Net Medical Home Initiative team has released the first version of the PCMH self-assessment tool.  The PCMH-A was created to help clinical practices gauge their progress in implementation of the Patient-Centered Medical Home change concepts.

The PCMH-A is an interactive PDF that can be downloaded, completed, saved and shared.  This tool may be further revised and changed.  ICIC will post news of any updated versions.

For questions about this tool, please contact the SNMHI team (Kathryn E. Phillips, MPH, Project Director, Safety Net Medical Home Initiative).
