Partnering in Self-Management Support: A Toolkit for Clinicicans

The MacColl Center, in collaboration with The Robert Wood Johnson Foundation's New Health Partnerships program and The Institute for Healthcare Improvement, authored "Partnering in Self-Management Support:  A Toolkit for Clinicicans".
This resource was created to provide busy clinical practices with a set of tested resources and tools to share with patients and families in the day-to-day management of chronic conditions. Practices that are just beginning to reorganize for patient-centered care now have a guide to a new way of working with their chronically ill patients. Experienced organizations and teams will find best practice activities and key changes, and links to helpful tools.
